Position Summary:
The Case Manager plays a critical role in connecting clients with resources and ensuring they receive comprehensive, trauma-informed support. This position involves assessing client needs, developing individualized care plans, coordinating services, and advocating for clients to achieve stability and wellness.
Key Responsibilities:
- Conduct thorough assessments to identify client needs and goals.
- Develop and implement individualized service plans.
- Coordinate referrals to community resources, healthcare providers, and social services.
- Monitor client progress and adjust plans as needed.
- Maintain accurate and timely documentation in compliance with agency and regulatory standards.
- Collaborate with multidisciplinary teams to ensure holistic care.
- Advocate for clients to access necessary services and overcome barriers.
Qualifications:
- Bachelor’s degree in Social Work, Psychology, or related field (Master’s preferred).
- Experience in case management or social services strongly preferred.
- Knowledge of community resources and trauma-informed practices.
- Strong organizational, communication, and problem-solving skills.
- Ability to work independently and as part of a team.
